I'm not sure if you are just new to it, but 'san' is just a way of respectfully addressing someone not from your immediate circle of friends or family. As such it has no place appearing after 'musuko'. Children refer to their mother as kaa-san because they are in a respected position in regards to their children, as such it is more or less required
