  K, I have a suggestion (I don't know if you want/need it)
  Putting a toothpick under his toenail, then making him kick a wall (hurts thinking about it for me)

  You could do a comfort thing, maybe some music playing softly and a few people rubbing him not in a sexual way, but maybe his back or neck and just carress him, make him relax, and people telling him it was time to wake up and go home, over and over.

  May i suggest a torture thingie??
  
  The Judas Cradle - victims would be placed on top of a pyramid-like seat, feet were tied to each other in a way that moving one leg would force the other to move as well - increasing pain.
  
  The triangular-shaped end of the Judas cradle was inserted in the victim's anus or vagina. This torture could last anywhere from a few hours to complete days.
  
  This doesn't kill the victim :3 But infection was common xD
